As you know we have made some changes to our application and intake process. ALL families MUST have an application on file (this is not optional).

When completing your application for the diaper bank it is required that you load proof of birth for ALL children to receive diapers.
Proof of birth:
• Birth certificate
• Insurance letter
• Hospital verification of birth letter
(Documents must have the child and parents (mom) name)
🚫 A pic of your family does not count as proof of birth.

🚫 Please do not load social security cards

We will be available 30 minutes before distribution starts to assist with applications. Applications can be done the day of prior to the start of distribution. You must bring in proof of birth to complete the application.

Diapers & wipes are given based on availability
● If your child is in a size NB, 1, 2, or 3 you will receive 30-40 diapers
● If your child is in diaper sizes 4, 5, or 6 or any size training pants you will receive 20-40 diapers
● 1 pack of wipes per family
● Due to the increase of children ages 4+ not fully potty trained, families will start to receive 20 training pants
● Please do not approach the diaper shelves and in an attempt to compare brands or certain packaging

Friendship & Piney Grove sites: you will receive only 20 diapers per child
Greene County: Greene County distributions are currently scheduled for the odd months of the year (Jan, Mar, May….)
Scheduling for the even months is based on the number of families that pick.
● Ex: if less than 10 families pick up in Jan, GC is not placed on the schedule for Feb.

Tuscaloosa:
● We ask that you come to 1 distribution (New Life or Forest Lake), in order to allow other families an opportunity to receive diapers.
● You will only be allowed to pick up for yourself. We are not doing multiple family pick-up until further notice (we apologize for the inconvenience this will cause, we have had too many families picking up for multiple people)

Our goal is to give directly to families. We reserve the right to refuse service to families picking up for multiple families. We ask that you try to make distributions to ensure that you are able to receive diapers. Diapers are given on a first come basis. We would like to be able to serve families that come, we are unable to do that especially when we have families picking up for multiple families whose children are in sizes that are currently “high needs”.

We will let you know if anything changes with the schedule.

****You will only be allowed to pick up for one family. You will not be able to pick up for yourself and another family. We apologize for the inconvenience and we understand that issues arise. When giving for multiple families to one person, it reduces what we have for families who actually come in person.****

❗️We cannot guarantee specific brand diapers or types of training pants.

When completing the application you agreed to the “Application Certification”. By receiving diapers from us you are agreeing to the “Application Certification” and you understand that we reserve the right to refuse service.

*I understand that Bottoms Up Diaper Bank & it’s sponsors provide donated diapers as received and cannot accommodate specific requests for styles or brands, As available, baby wipes may be distributed. We cannot make any guarantees of availability.

Diaper Bank & it’s partners for the sole purpose of distributions to families in need of diapers. Bottoms Up Diaper Bank and partners does not take any responsibility or liability for products it is distributing.
*I certify that the information given on this application is accurate to the best of my knowledge. I certify that the diapers I receive will be solely for the use of the children named above and should not be sold or distributed to other people. All diapers, training pants, wipes, & baby essentials are donated for the purpose of distribution to families in need of diapers.
*The information is being requested to help us request grants for funding, distribution sites, and for coordinating with agencies and organizations to best serve the diaper bank. This information will only be used for that purpose.
